Registration, Waiver, & Check-in Information
Registration
Every walker must register with NAMI of Greater Chicago. This can be done by sending name, address, phone, and email information to our office via mail: 1536 W. Chicago Ave., Chicago, IL 60642, phone: (312) 563-0445, fax: (312) 563-0467, email: namiwalkschicago@gmail.com, or by submitting the information online: www.namigc.org (Click on NAMIWalks logo, Walker registration).
Walkers can register themselves, or you, as Team Captain, can register them. As Team Captain, your task is to ensure that every walker on your Team knows they must register prior to the start of the walk.
Waivers—REQUIRED!
Every walker (Team Captains included) MUST SIGN A WAIVER OF RELEASE AND LIABILITY (required by the Chicago Park District). To speed up registration on the day of the Walk, please make every effort to return signed waivers to NAMI-GC by Thursday, September 28th, 2008. The waiver must include the text below and must be signed and returned to NAMI of Greater Chicago, 1536 W. Chicago Ave., Chicago, IL, 60642, or faxed to (312) 563-0467. The waiver can also be printed off of our website: www.namigc.org.
Waiver of Release and Liability:
I hereby waive all claims against NAMI, NAMI of Greater Chicago and its NAMI Affiliate Walk Partners, City of Chicago Park District, Sponsors, or any personnel for any injury that I might suffer in this event. I attest that I am physically fit and prepared for this event. I grant full permission for organizers to use photographs of me and quotations from me in legitimate accounts and promotions of this event.

Check-in (the Day of the Walk)
Each walker must come to the large canopy tents marked “Check-in,” give their last name, and check-in prior to the Walk’s 10 AM start time. We recommend that walkers arrive at 9 AM to catch the live music, booths, festivities, friends, and to check-in.
